lMedical care for older adults and the frail elderly is problematic due to fewer primary care doctors and lack of geriatric expertise in most community settings.
lWe are creating an innovative, patient owned, non-profit, primary care medical support process.
lIt will pioneer geriatric “guided care,” transition support, care management services and patient advocacy.
lVolunteer peer advocates will track individual members through rehabilitation processes, from hospital discharge through nursing centers and finally into the home environment. Our intent here is to augment the process of wellness by backstopping the individuals recovery from a health related incident.
